Dermatology records, photographs of lesions, biopsy if performed, and treatment history including biologics (Humira, Stelara, Cosentyx for psoriasis).
Common misconceptions
Psoriasis responding to biologics usually doesn't qualify. Severe treatment-resistant cases and erythrodermic variants qualify more readily.
